The Board of Trustees of Crook County Medical Service District seeks to appoint an interim trustee to fill a vacancy in an at-large position on the Board. The interim trustee will serve until a trustee is elected for the position in November 2026. The interim trustee must be a qualified elector residing in Crook County and must not be employed by Crook County Medical Services District. Short-Term Transitional Care

Recovering from an illness, injury, or surgery? Take advantage of short-term transitional care at Crook County Medical Services District (CCMSD). If you’ve been hospitalized and no longer require inpatient care but aren’t quite ready to return home, transitional care provides the support you need. Even if your surgery took place at another hospital, you can return to your community and continue your recovery before transitioning home.

About Short-Term Transitional Care

Transitional care is offered at Crook County Hospital and our skilled nursing facility through our Swing Bed Program—a short-term step between hospital care and home. Our program provides acute medical care and short-term stays for patients requiring medical treatment for acute or chronic illnesses. If long-term treatment is needed, patients may remain in their room and seamlessly transfer to skilled swing bed services.

Skilled Swing Bed Services

Our Swing Bed Program at Crook County Hospital provides a comfortable and individualized recovery environment. Patients receive 24-hour nursing care and access to skilled therapy services while staying close to home.

Services include:

  • Skilled nursing care
  • Short-term rehabilitation, including physical, occupational, and speech therapy
  • Long-term IV antibiotic treatment
  • Advanced wound care

We also welcome patients who were hospitalized far from home to transfer to CCMSD for continued skilled care and rehabilitation in their own community.

Nursing Care at CCMSD

At CCMSD, we take pride in offering high-quality, patient-centered care with a compassionate and community-oriented staff. Our low patient-to-nurse ratio ensures personalized attention and the best possible care experience. Our team includes highly qualified nurses with emergency certifications to meet a variety of patient needs.

Updated Comfort Care Room

For patients requiring end-of-life nursing care beyond what can be provided at home, we offer two recently renovated private comfort care rooms. These rooms are designed to provide a peaceful, home-like setting with:

  • A Keurig coffee maker for family comfort
  • A couch and a hide-a-bed for loved ones staying overnight
  • Wardrobe storage for personal belongings

Discharge Planning

Your length of stay in transitional care will depend on your condition and progress. Your care team will work closely with you and your family to track your recovery and ensure you have the support and resources needed for a smooth transition home. A family care conference will be held to discuss your next steps and the best options for ongoing care.
